This is a personal site. The only place it collects information from you is the contact form. This page explains what that form collects, where the data goes, and how to have it removed.
What the contact form collects
When you submit the contact form you provide your name, email address, and message (required), and optionally your organization, the role you're writing about, and your location. The form also automatically records limited technical metadata for security and record-keeping: your IP address, your browser's user-agent string, and the country inferred from your IP address.
Where submissions go
Submissions are stored in a hosted database (Supabase) and may also trigger an email notification (sent via Resend) and an internal monitoring alert, so the inquiry can be reviewed and answered. These providers process the data solely to operate this contact feature. Basic spam protection uses a hidden honeypot field — no additional information is collected from you for it. Your contact data is not sold, and it is not shared with anyone other than these providers.
Requesting deletion
You can ask for your contact data to be deleted at any time by emailing contact@drjeremytabernero.org.
Cookies and tracking
This site sets no advertising cookies and uses no advertising trackers.
This is not a patient portal. Please do not submit clinical questions, health information, or symptoms through the contact form. For questions about your own care, contact your treating clinician directly. In an emergency, call 911.
